Position Information The Change Management Specialist is a member of the Local Delivery Group (LDG) Program and plays a key role in the successful implementation and adoption of provincial digital services - specifically eReferral and Central Intake - across the Acute Care sector within the Ontario East Region.
Applying a structured change management approach and methodologies, the incumbent promotes partner awareness and education of the solutions and benefits of digital services. This role acts as a primary point of contact and visible coach to physicians/clinicians and administrative staff and supports meaningful engagement and participation to ensure local clinical, operational and digital needs are incorporated into both process and system design.
The incumbent supports large-scale, multi-site digital health transformation, to align work across digital health priorities and harmonize the models of change, facilitate corporate change and communication of the digital services, and collaborate with regional and provincial partners to advance adoption and benefits realization.

Days, 8 hour shifts.

Basic Requirements
- Four (4) year Undergraduate degree in Administration, Healthcare Administration, Business Administration, IT or a related discipline;
- Five (5) years experience in supporting digital health initiatives or within a healthcare capacity;
- One (1) year of leadership experience;
- Demonstrated experience in supporting large scale, multi-site digital health or technology transformation initiatives;
- Previous experience with enabling technologies and digital health within Ontario;
- Strong understanding of change management principles, methodologies and best practices, with the ability to lead, facilitate and support organizational change initiatives;
- Excellent relationship building, collaboration, engagement, conflict resolution, negotiation and mediation skills;
- Strong communication (written and oral),
active listening and interpersonal skills
- Strong analytical, systematic problem-solving and critical thinking skills, with the ability to challenge existing approaches, identify innovative solutions, and maintain a high level of attention to detail;
- Ability to manage frequent, competing demands of multiple stakeholders in a diverse, fluid setting
- Demonstrated ability to deliver training on digital health technologies
- Sound technical knowledge to support digital interoperability services across different referral management solutions
- Well versed in virtual meeting tools (Zoom, Teams), computers and basic troubleshooting
- Demonstrated track record as a change agent, successfully leading and influencing change initiatives to achieve organizational objectives and sustained adoption.
Preferred Qualifications
- Change Management Professional certification (e.g,. Prosci/ADKAR or equivalent);
- Project Management training/certification;
- Experience implementing eReferral digital tools and working with EMRs and/or HIS (Ocean eReferral, Accuro, OscarPro, PS Suite, Epic, Oracle preferred);
- Experience in data reporting/management, evaluation, research and measurement of adoption;
- Understanding of clinical processes in acute/specialist care;
