JOB TITLE: Term Full-Time Flex Assisted Living Support Worker
JOB SITE: Holly House
REPORTS TO: Assisted Living Team Lead & Mental Health Programs Manager
JOB SUMMARY:
The Assisted Living Support Worker provides support services and case management to clients with mental illnesses in a home setting. The Assisted Living Support Worker will assist clients with a variety of day-to-day physical, emotional, and social supports, advocacy, life skills assistance, information, resources, and demonstrations to support their stable housing and achievement of personal goals.
EMPLOYEE CATEGORY: Full-Time, Term (Until September 30, 2027)
SALARY RANGE: Grid 34 – HEU – Community Subsector
NUMBER OF VACANCIES: 1 (one)
HOURS OF WORK: Tuesday-Saturday | 11:00-19:00
QUALIFICATIONS:
- A diploma or degree in Human Services OR an equivalent combination of relevant education, training and experience, including lived experience
- 3+ years of recent related work experience working with vulnerable populations in a residential setting. Experience in Assisted Living is preferred.
- A valid Class 5 BC driver’s license.
- A reliable vehicle to use for work purposes
- Driver’s insurance & abstract
- Current First Aid certification
- Food Protected certification
- Naloxone certificate
- Recovery-Centred Clinical Systems (RCCS) training
- Provincial Violence Prevention certification an asset
- Successful clearance of a criminal record and vulnerable sector check
KEY DUTIES AND RESPONSIBILITIES :
- Maintains the daily functioning and safety of the residential program: ensuring adherence to schedules, identifying safety hazards, cleaning, responding to client requests/needs, ensuring basic needs are met, and reporting all concerns appropriately.
- Participates in person-centred case planning in collaboration with clients, colleagues, clinical partners, as well as other community resources and family as appropriate.
- Supports the mental health wellbeing, recovery, and psychosocial support of clients in the program, including crisis intervention.
- Advocates for and supports clients by problem-solving in areas such as housing retention, financial assistance, accessing community resources, and pursuing their personal goals (e.g. employment/volunteering, education and training, social and recreational activities).
- Observes clients,
identifies and assesses potential emergency situations, and develops short-term strategies to deal with such situations. Reports medical, behavioural, and other problems as required. Provides feedback and input regarding clients’ needs, performance, and progress.
- Provides supportive counselling, demonstrations, modeling, and behaviour management to clients to assist with the development of physical, social, emotional, and life skills such as maintaining personal hygiene, housekeeping, meal planning and preparation, meeting financial obligations, making and keeping appointments, and interpersonal skills.
- Administers medication to clients and provides medication reminders, in accordance with established policy.
- Identifies available social, economic, recreational, and educational services and resources in the community that will meet clients’ needs. Provides clients with related information.
- Facilitates clients’ physical, recreational, educational, and employment needs by performing duties such as arranging for clients to be transported to programs/services, assisting clients to attend appointments, and implementing programs.
- Carries out administrative transactions such as petty cash, client accounts, collecting rents and money from clients for personal items, and making deposits and withdrawals.
- Assists clients with activities of daily living such as feeding, lifts and transfers, grooming, and toileting.
- Ensures sufficient client nutrition by supporting clients to prepare meals according to their level of independence. Performs food preparation and service as necessary.
- Maintains accurate, up-to-date and appropriate records; ensures all data is current and captured in relevant databases and formats.
- Completes and maintains current files, case notes, daily logs, progress reports, intake/ transfer/ discharge documentation, and relevant client information.
- Receives client feedback, inquiries, and complaints,
and responds as required.
- Completes agency required staffing documentation such as scheduling, leave requests, and expense claims.
- Performs other related duties as required
SKILLS AND ABILITIES:
- Ability to engage in empowering, respectful, and non-judgemental rapport building with people from diverse backgrounds and life experiences
- Demonstrated ability to establish and maintain constructive working relationships with clients, co-workers, supervisors, community groups, government agencies, and other partners.
- Knowledge of theory, principles and practices of psychosocial supports, trauma-informed care and harm reduction strategies.
- Ability to communicate effectively, both verbally and in writing
- Physical ability to lift up to 50lbs and perform the duties of the position
- Ability to work independently and in cooperation with others.
- Ability to plan, organize and prioritize.
- Ability to observe and recognize changes in clients
- Home management skills
- Demonstrated cooking skills, with the ability to mentor or train others in meal preparation
- Ability to instruct
- Ability to analyze and resolve problems: using judgement within scope and knowing when to ask for assistance/consultation when needed
- Strong case management skills using person-centred planning.
- Systems navigation and advocacy.
- Knowledge of mental health conditions, substance use, systemic barriers, and their impacts on individuals and the community.
- Ability to operate related equipment.
