Currently we have an exciting opportunity for a Transplant Liaison Nurse within the Nephrology Program.
As an integral team member of the Nephrology Program, there is an opportunity to participate within the Program by utilizing a chronic disease prevention and management (CDPM) and self-management approach and philosophy, as the Program continues to achieve the deliverables under the Ontario Renal Network, and strives to achieve the targets of the Ontario Renal Plan II:
- Empower and support patients to be active in their care
- Integrate patient care throughout the kidney care journey
- Improve patient access to kidney care
The Transplant Liaison Nurse, working within the partnership of Trillium Gift of Life Network and the Ontario Renal Network will be committed to enhance access and improve patients’ experiences with kidney transplant; with a focus on living kidney donation Employment Status: Temporary Full-time Duration : February 2027Hours of Work: Available to work rotating shifts including weekends, day and evening shifts.Location: Humber River Health – Wilson Site cross site coverage may be requiredLabour Group: ONAReporting Relationship: Clincial ManagerHourly Rate: 41.15 - 58.98
Responsibilities:
The Transplant Liaison Nurse establishes and promotes a liaison relationship between the Multi Organ Transplant Centers and renal patients and families. During the pre-transplant phase the Transplant Liaison Nurse ensures the patients’ readiness for transplant by confirming required testing is being completed as needed, Transplant Referral packages are complete and up to date, social support is intact, and other tasks as needed.
The Transplant Liaison
Nurse will also track the progress of the transplant work-up and ensure timely and consistent follow up. In collaboration with referring transplant centers, the Liaison Nurse applies competent clinical knowledge and skill to achieve quality care outcomes.
Qualifications:
- Current RN registration with the College of Nurses of Ontario
- Current C-Neph Certificate
- Minimum two years of Nephrology experience; work experience in MCKC is an asset
- Minimum two years of recent work experience in renal replacement therapies education is required
- Experience in developing, leading and providing large event information sessions and focus groups
- Comprehensive understanding of the Ontario Renal Network Transplant Ambassador Program
- Experience educating, assessing, and working up pre-dialysis patients within Multi Care Kidney Clinic
- Experience working with Regional Transplant Programs
- Experience collecting and reporting ORN transplant data
- Proficiency in Quality Improvement methodology
- Demonstrated good interpersonal and communication skills with an ability to work with a multidisciplinary team, including other health team members, patients, family members and visitors
- Ability to be flexible and effectively work in a busy environment with changing priorities and tight deadlines
- Ability to prioritize and complete assignments in a reasonable length of time
- Demonstrated sound judgment, good observation and patient assessment skills, tack and empathy
- Demonstrated good patient/family teaching skills
- Proven ability to perform a leadership/preceptorship role
- Physically capable of performing the essential duties of the position
- Demonstrated understanding of and commitment to Humber's comprehensive safety programs and practices, including staff patient, and environmental safety, and participation in regular in-service education in this area; and
- Demonstrated initiative in improving work skills and processes
- Ability to perform all physical demands required in the dialysis unit
- Basic computer skills and Meditech experience is an asset
- Excellent attendance and discipline free record required
