Job Summary The Case Manager at the Transition House is responsible for overseeing day-to-day operations to ensure the delivery of quality services and effective case management for residents. Reporting to the Transition House Case Manager Lead, the Case Manager works closely with all Transition House staff and other Crossroads employees to promote best practices and trauma-informed care within the shelter. This role includes supporting clients in navigating systems such as mental health and emotional support, housing, employment and income, childcare, the legal system, and more.
Responsibilities
- Provide individualized case management support to residents, ensuring services are trauma-informed, anti-oppressive, and client-centred.
- Collaborate with residents to develop, implement, and regularly review personalized plans addressing safety, housing, health, financial, legal, and other needs.
- Coordinate referrals, advocate with external partners (housing, healthcare, legal, social services), and connect residents to community resources.
- Hold regular case meetings with residents to track progress, address barriers, and celebrate successes.
- Participate in case consultations and reviews with the Transition House Manager and other program teams (Child & Youth, Jade, etc.) to ensure integrated, holistic support.
- Identify service gaps or emerging needs and raise them with the Transition House Manager to address them and develop resources.
- Respond promptly to urgent or complex resident needs, consulting with management as required.
- Ensure accurate, timely, and confidential documentation of resident interactions, case notes, and service plans, in compliance with agency policies and professional standards.
- Contribute to a respectful, supportive, and inclusive environment where residents feel safe, empowered, and heard.
- Support community clients as needed. For example, to complete GBV applications.
- Be on call as needed.
- Any other duties related to supporting transition house residents’ well-being and progress, including planning social events.
Qualifications
- Education: Diploma or Degree in Social Work, Psychology, Human Services or a related field; relevant or lived experience.
- Experience: Minimum 1 year in frontline work.
- Knowledge: Strong understanding of gender-based violence, trauma-informed care, and anti-oppressive practice.
- Communication: Strong verbal and written communication skills in English (French bilingualism preferred)
- Data Management: Experience in case notes, preparing reports, collecting statistics and handling confidential client information.
- Flexibility: Ability to work occasional evenings or weekends as needed.
- Other Requirements:
- Satisfactory vulnerable sector and criminal record check.
