The Oxford County Community Health Centre (OCCHC) exists to improve the health and well-being of individuals and families who face barriers to accessing health, housing, and social supports. We are seeking a Transitional Housing Worker- Youth and Family Programs to join our Housing Team in Woodstock.
Overview – the incumbent will:
Work with County of Oxford partners to determine selection of clients for the Youth or Family House based upon Coordinated Access / By-Name List (BNL) criteria
Meet with prospective Transitional Housing clients to complete intake procedures – assess & triage client needs, & determine readiness for transitional living
Assist clients to develop basic life skills through wraparound supports, including coaching & goal planning; building motivation & confidence to achieve goals & stability; instilling a sense of emotional & social belonging; developing creative group programs; house seeking; attending appointments; & establishing trusting relationships, to name a few
Uphold daily operations at both sites e.g. guest guidelines, health & safety, room checks, maintenance, conflict mediation, discharges & move-in’s, supplies coordination
Build robust & productive relationships with community partners & landlords
Navigate community resources, & transition clients toward permanent housing
Key Responsibilities
Receive referrals, complete intake and assessment processes, determine program eligibility, and prioritize client needs.
Develop individualized support plans with clients that build life skills, independence, motivation, and personal goals.
Provide trauma-informed, recovery-oriented support to individuals experiencing mental health and substance use challenges, including crisis intervention, de-escalation, emotional support, harm reduction education, and referrals to appropriate services.
Support clients in accessing healthcare,
income supports, housing, employment, education, and other community resources.
Maintain daily operations of the transitional housing program, including room checks, move-ins and discharges, client orientation, conflict resolution, health and safety practices, household supplies, and documentation.
Support clients with housing searches, landlord engagement, appointments, and successful transitions into permanent housing.
Build team-oriented relationships with community partners, landlords, healthcare providers, and social service organizations.
Maintain accurate documentation and participate in case conferencing, team meetings, and continuous quality improvement initiatives.
Requirements
Completed post-secondary education in social services & a current Member of a regulatory college in good standing, such as the Ontario College of Social Workers and Social Service Workers (OCSWSSW), OR completion of a Child and Youth Care Program Diploma
Considerable experience addressing the needs of individuals and families experiencing homelessness, or those at risk of homelessness
Understanding of community agencies and a wide cross-section of service providers – preference will be given to candidates with knowledge of Oxford County
Demonstrated understanding of the Child, Youth and Family Services Act The following would be considered assets:
Experience working within a transitional or supportive housing environment.
Training in suicide intervention, crisis response, de-escalation, motivational interviewing, or non-violent crisis intervention.
Valid Ontario Class G driver's licence and access to reliable transportation, if travel within Oxford County is required.
Compensation
Hourly Wage: $28.00 to $33.80 per hour
4 weeks paid vacation

Hours of Work The regular schedule is Monday to Friday. Work schedules may vary to meet operational requirements and needs of the programs.
The successful candidate may also participate in a shared on-call rotation. On-call assignments will be scheduled in advance whenever reasonably possible and compensated in accordance with OCCHC policy and applicable employment standards.
