CarePartners is seeking a Personal Support Manager to coordinate and supervise Community Health Services, ensuring high-quality patient care and team mentorship. The role involves leading the Community Support Workers, developing care plans, and aligning with Ontario Health atHome programs.

The candidate will monitor RHPA compliance, promote CHS services, and guide health and safety practices. A RN or RPN with CNO registration is required, plus travel readiness and CPR certification.
