We are seeking a family physician with an interest in medical education to support Primary Care delivery while contributing to the teaching and supervision of residents and medical students.The successful candidate will be expected to obtain and maintain faculty appointment with the Department of Family and Community Medicine at the University of Toronto.ResponsibilitiesPractice in accordance with the College of Physicians and Surgeons of OntarioProvide routine and preventative healthcare services to a patient panel of all ages with varying healthcare needsDevelop personalized treatment plans including medication management, lifestyle recommendations and referrals to specialists as requiredManage and monitor chronic conditions such as diabetes, hypertension and respiratory diseaseMaintain accurate and up-to-date electronic health records to facilitate efficient and effective patient care.Demonstrated experience in family medicine/primary care settingsAssess, plan, implement, and evaluate the needs of patients in collaboration with the interprofessional teamPerform patient assessments, medical history, physical, and psychosocial assessments.Diagnose, treat, prescribe, order and interpret diagnostic imaging reportsOffer psycho-social assessments, service coordination, therapeutic and/or supportive counselling.Implement appropriate health promotion, disease prevention and chronic disease self-management strategies for patients and familiesRefer to various clinical resources, liaising between hospital, specialists, and community servicesEnsure linkages and collaborative transitions of care to primary care, community outreach, and education supportSupport evaluation and quality improvement activities to assess impact and outcomes and determine strengths, opportunities, and future directions of the IPCT servicesRequirementsCurrent registration and a member in good standing with the College of Physicians and SurgeonsMembership and Certification with the Canadian College of Family Practitioners (CCFP) is preferredStrong knowledge and understanding of primary health care, chronic disease prevention and self-management, preferably in a community setting2-3 years recent clinical work experience as a Physician with community experience preferred.Excellent verbal and written communication skillsStrong interpersonal, leadership,
