Key Responsibilities:
• Maintain up-to-date participant files and daily process notes




• Collaborate with clinical teams on incident reporting
• Engage participants in goal setting and personal planning
• Monitor and report on participant health care
• Conduct duties as assigned by management

Requirements:
• 18 years of age or older
• Minimum two years of direct support experience
• Valid Standard First Aid / CPR certification
• Clear criminal record and abuse registry checks
• Post-secondary education in Human Services preferred
