The Canadian Mental Health Association is seeking two passionate individuals for Case Manager roles focused on improving the well-being of clients. You'll assess their unique situations, aid in crisis planning, and connect them to critical resources. Your compassionate approach will help empower clients to lead fulfilling lives, while adhering to CMHA's mission and values.

Key Responsibilities:
• Provide case management for clients with serious mental health conditions
• Assist clients in areas like food security and harm reduction




• Assess client needs and create tailored support plans
• Monitor client service utilization and progress
• Collaborate with community organizations and resources

Requirements:
• Degree in Human Services or a related field
• Three years of experience in mental health support
• Current registration with OCSWSSW preferred
• Certification in First Aid/CPR and ASIST within 3 months
• Valid driver's license and reliable vehicle
