Become a vital team member at SLCS as a Full-Time Case Worker in our Supported Independent Living (SIL) Program. Support adults with disabilities and mental health needs, assisting them in independent living skills and resource connection.

SLCS is searching for dedicated Full time Case Workers for our SIL Program. This role involves developing individualized case plans for clients, providing essential life skills education, and conducting crisis intervention when necessary. Your ability to assess client needs and connect them with community resources will be crucial in facilitating their independence.

Key Responsibilities:
• Develop individualized case plans for clients in need
• Facilitate connections to community resources and supports
• Educate clients on essential life skills for independence




• Assess client risk and needs regarding safety and care
• Document and prepare case notes regularly

Requirements:
• Post-secondary education from an accredited institution
• 1-3 years experience with developmental disabilities
• Experience with de-escalation strategies for aggressive behaviors
• Valid G Class Driver’s License and reliable vehicle
• Standard First Aid/CPR certification
