Job Description : The CDM Clinical Nurse Educator reports to the Manager Clinical Integration and works with an interdisciplinary team with a chronic disease prevention and management focus. The CDM Clinical Nurse Educator combines in-depth clinical expertise, with the principles of teaching and learning, in the provision of self-care management for clients, families, health professionals and students in group and individual settings.

Experience
3 years clinical experience with patients/families with chronic disease
Education
Bachelor of Science in Nursing (BScN)
Licenses
Valid Class 5 driver's license
Other Education and Training
Basic Life Support Certificate (BLS)
Nursing diploma is acceptable
Demonstrates word processing skills using Microsoft Office programs
Advanced - Communication skills
Basic - Facilitation skills
Basic - Assessment skills
Knowledge and Abilities
Ability to develop, evaluate and modify programming provided to clients
Communication skills to transfer knowledge/skills
Ability to develop and deliver formal education programs
Ability to adapt to changing conditions
Demonstrates ability in teaching, facilitation and group activities
Ability to competently use computers and software applications
Other Information
May consider candidates able to obtain qualifications within 18 months
Must have obtained one or more of the Chronic Disease Management certifications
