you will utilize Children's Oncology Group guidelines to develop personalized follow-up care plans.Key Responsibilities:Work at the Survivorship Clinic with young cancer survivorsCreate surveillance recommendations using established guidelinesCollaborate with primary care providers on patient careInitiate referrals and follow up on surveillance investigationsProvide in-person and occasional virtual clinic visitsRequirements:Licensed to practice medicine in OntarioExperience in acute care and cancer care preferredStrong interest and knowledge in cancer patient supportExcellent interpersonal skills and teamwork abilitiesResponsible and organized in a clinical environmentEmpower pediatric survivors by addressing their comprehensive health needs in a supportive UHN environment.
