Sault Area Hospital in Sault Ste. Marie, Ontario, is seeking a Reactivation Worker to support the Geriatric Emergency Management (GEM) Program on a temporary part-time basis.
The role focuses on early mobilization, functional reactivation, and discharge planning to reduce deconditioning and improve transitions of care. Under delegated authority of the GEM team, including RNs and Social Workers, you will implement therapeutic activities and collaborate to maintain patient independence and protected ED
