Key Responsibilities: • Lead resident move-in, transfer, and move-out processes • Provide counseling and assess psychosocial needs • Develop and implement individual social work services • Support advanced care planning conversations • Manage correspondence and feedback tracking in homes

Requirements: • Registered Social Worker per provincial regulations • Minimum three years’ healthcare or social services experience • Knowledge of Long Term Care Act preferred • Strong communication and organizational skills • Experience in customer service or hospitality is an asset
