If so, the position of Palliative Care Resource Nurse (PCRN) with the North Zone Specialty Programs - Palliative Care Team could be for you.As a PCRN, you will play a key role in supporting Continuing Care clients throughout the North Zone. You will be a member of a mobile/virtual interdisciplinary team providing consultation services for palliative and end-of-life care clients. Using a client and family-centered approach, you will work with local clinicians and teams to provide specialized assessment, care planning and follow-up in a consultative, limited direct-service model.You will participate in assessments, case reviews and consultations, liaise with Continuing Care, Home and Community Care, Acute Care and community partners, and identify client and system barriers while supporting system navigation. This position supports clients across the North Zone and is based out of Beaverlodge. The role can accommodate working from home; however, you must be willing and able to travel to locations within the North Zone, approximately two hours from your home site.Candidates must be able to work independently in varying environments and weather conditions and carry a medical bag weighing approximately 25-30 lbs. In addition to a competitive rate of pay, AHS currently has a Remote Retention Allowance in place to an annual maximum of $3,000.
