The Scarborough Academic Family Health Team (SAFHT) is seeking a qualified and experienced individual to join our team as a Nurse Practitioner (NP). SAFHT has 2 new NP roles that will report to the Executive Director and/or manager and will work our interdisciplinary team. The successful candidates will primarily work within one of SAFHT’s affiliated physician offices and collaborate closely with the supervising physician and interdisciplinary team to provide comprehensive, patient-centred primary care services. The NP may be required to travel between SAFHT sites as operationally required.

Working to full scope of practice, the Nurse Practitioner provides comprehensive primary health care services to patients across the lifespan. The NP independently assesses, diagnoses, treats, monitors, and manages acute and chronic health conditions while collaborating with affiliated physicians and the interdisciplinary team to ensure timely, accessible, evidence-informed care.

The Nurse Practitioner primarily provides care to patients of SAFHT affiliated physicians, supporting continuity of care and timely access to primary care services. The NP practices autonomously within the legislated scope of the Registered Nurse (Extended Class) role while collaborating closely with physicians and other health professionals to meet the needs of individual physician practices and their patients.

The NP integrates health promotion, disease prevention, chronic disease management, episodic care, same-day access, urgent care, and care coordination to improve patient outcomes and support continuity of care. The successful candidate will exercise advanced clinical judgment, provide leadership in evidence-informed primary care practice, and contribute to SAFHT's academic and quality improvement initiatives.

The SAFHT is governed by a Mixed Board (i.e. Physicians and Community Members) and is an integrated group of health care providers that work collaboratively as part of a family health team. Our interprofessional care team currently includes 29 affiliated physicians, nurse practitioners, registered nurses, registered dietitians, a case manager, social workers, chiropodists, and several administrative staff. Originally formed in 2007, the SAFHT serves patients by providing them with timely access to a comprehensive range of services and programs aimed at managing and preventing disease and improving health status. Including our SAFHT affiliated physician group locations, we deliver care across 8 locations in the Scarborough and Eastern Toronto geography and engage in teaching learners in medicine and other health disciplines. Role Responsibilities:

Working collaboratively with affiliated physicians and the interdisciplinary team, the Nurse Practitioner:

- Provides comprehensive primary care services to patients of affiliated physicians while practicing to the full legislated scope of the Registered Nurse (Extended Class).
- Assist with onboarding new patients
- Performs comprehensive health assessments, including complete medical histories and physical examinations.
- Independently assesses, diagnoses, investigates, treats, monitors, and manages acute illnesses, episodic concerns, and chronic diseases across the lifespan.
- Orders, performs, interprets, and communicates laboratory investigations, diagnostic imaging, and other diagnostic tests within legislated scope of practice.
- Prescribes medications, therapeutic interventions,



and medical devices in accordance with applicable legislation and professional standards.
- Develops, implements, monitors, and evaluates individualized treatment plans.
- Provides ongoing management of patients with chronic diseases including diabetes, hypertension, COPD, asthma, heart failure, chronic kidney disease, osteoporosis, and other common primary care conditions.
- Performs procedures within NP scope of practice and individual competency, including immunizations, injections, wound care, ear irrigation, Pap testing, suture and staple removal, and other office-based procedures.
- Perform well-baby visits
- Perform and assist in performing pelvic examinations.
- Provide geriatric care
- Perform house calls when needed.
- Provides same-day and urgent appointments as required to support patient access and the operational needs of affiliated physician practices.
- Provides preventive care including cancer screening, cardiovascular risk assessment, immunizations, smoking cessation support, sexual health, healthy lifestyle counselling, and disease prevention initiatives.
- Provides patient and family education to support self-management, informed decision-making, and healthy behaviours.
- Coordinates referrals and transitions of care with specialists, hospitals, community agencies, and other health care providers.
- Collaborates with physicians and other interdisciplinary team members regarding complex patient care and shared management.
- Documents assessments, diagnoses, treatment plans, prescriptions, and patient encounters accurately and in a timely manner within the TELUS Practice Solutions Suite (PSS) electronic medical record.
- Participates in interdisciplinary case conferences, program development, quality improvement initiatives, and organizational planning.
- Participates in the education and supervision of Nurse Practitioner students and contributes to SAFHT's academic mandate.
- Maintains patient confidentiality and complies with applicable legislation, professional standards, infection prevention and control requirements, and organizational policies.

Accountability

The Nurse Practitioner reports administratively to the Executive Director and/or Manager of SAFHT.

The Nurse Practitioner practices independently, works collaboratively with physicians and is accountable for their own clinical practice in accordance with:

- College of Nurses of Ontario Practice Standards
- Nursing Act, 1991
- Regulated Health Professions Act, 1991
- Relevant Ministry of Health legislation, regulations, and policies
- SAFHT policies and procedures

Qualifications

- Current registration in good standing with the College of Nurses of Ontario as a Registered Nurse (Extended Class) [RN(EC)].
- Completion of an approved Primary Health Care Nurse Practitioner education program leading to registration with the College of Nurses of Ontario as a Registered Nurse (Extended Class).
- Demonstrated knowledge of comprehensive primary health care across the lifespan.
- Strong clinical assessment, diagnostic reasoning, critical thinking, and evidence-informed decision-making skills.
- Demonstrated ability to work independently while collaborating effectively within an interdisciplinary team.
- Excellent communication,



organizational, leadership, and interpersonal skills.
- Experience using electronic medical records; proficiency with TELUS Practice Solutions Suite (PSS) is considered an asset.
- Proficiency with Microsoft Office applications.
- Current Basic Life Support (BLS) certification.
- Demonstrated commitment to continuous quality improvement, evidence-informed practice, and lifelong learning.

Assets

Preference may be given to candidates with one or more of the following:

- Previous experience as a Primary Health Care Nurse Practitioner in a Family Health Team, Community Health Centre, academic family medicine clinic, or other primary care setting.
- Experience independently managing patients with acute illnesses and multiple chronic diseases.
- Certified Diabetes Care and Education Specialist (CDCES) designation.
- Experience performing women's health procedures, minor office procedures, and other interventions within NP scope of practice.
- Experience participating in quality improvement, accreditation, or program evaluation initiatives.
- Experience preceptoring or supervising Nurse Practitioner students.
- Ability to communicate in languages reflective of Scarborough's diverse communities.

Other Responsibilities

- Collaborates closely with affiliated physicians to provide coordinated, patient-centred primary care.
- Supports same-day access and continuity of care based on the needs of affiliated physician practices.
- Assists in the planning, implementation, and evaluation of health promotion, disease prevention, and chronic disease management initiatives.
- Participates in quality improvement initiatives, accreditation activities, organizational committees, and program evaluation as a member of the SAFHT interdisciplinary team.
- Supports the education, supervision, and evaluation of Nurse Practitioner students during clinical placements.
- Maintains collaborative relationships with hospitals, specialists, community agencies, Ontario Health Teams, and other health system partners.
- Maintains professional competence through continuing education and ongoing professional development.
- Provides timely reports and statistical information as required.
- Performs other duties as operationally required.
