Summary Under the direction, accountability and guidance of the Mental Health and Addictions Manager, the Mental Health and Addictions Intake/Care Coordinator is the main point of contact for mental health and addiction services and will facilitate triage and screening of all individuals seeking service and determine type of services required. The Intake/Care Coordinator is familiar with other services and resources in the community and is proficient at system navigation. Case consultation, case conferencing and advocacy with community partners and services are an integral part of the role. Due to the specialized nature of this position the Intake/Care Coordinator needs to have an in depth understanding of the treatment pathways and risks associated with the use of opioids. The Intake/Care Coordinator will collaborate, consult and send/receive referrals from relevant internal and external service providers.
Intake
Conduct intakes and assessments with individuals seeking MH&A; services, including suicide risk assessments and safety plans as necessary
Triage and assign individuals seeking services to most appropriate providers on the MH&A; team
Collaborate with referring agency to receive further information as needed and update on status of referral
Perform administrative tasks associated with intake including sending waitlist letters to clients as appropriate
Monitor status of waiting lists for MH&A; programs
Counselling
Provision of supportive counselling to individuals seeking services from the MH&A; program while on waiting list for services from team clinicians and/or during times of staffing vacancies as needed
Maintain a comprehensive understanding of crisis management and the impact of trauma on mental health and addictions issues
Participate in group consultation with Mental Health & Addictions team at Noojmowin Teg Health Centre
Navigation
Coordinate and facilitate access to traditional healing and other cultural programming
Refer clients to external service providers and programs as needed
Liaise with community agencies and supports
Community/Program Development
Participate in promotion services related to mental health and addiction program
Attend and participate in community and organizational committees as directed
General Description of Service Responsibilities
Completion of charting and data collection within the Electronic Medical Record utilized by Noojmowin Teg Health Centre
Compilation and completion of all reports as required by funders, and other data (including quality health indicators) as developed by the organization
Adherence to policies and procedures
Membership with and adherence to guidelines of regulatory college
Adherence to traditional protocols and cultural sensitivity to First Nation where services are provided
Complete all required leave request documentation (i.e., annual leave, professional development, certified sick leave, uncertified sick leave, over‑time leave, etc.) in a timely manner
Complete relevant professional development and training as provided by Noojmowin Teg, the funder, or legislative requirement
Attend staff meetings, strategic planning, wellness activities, annual general meeting, and other team/group requirements
Requirements Mandatory
Minimum of Bachelor of Social Work degree; Social service/health related degree with experience may be considered
Registration with the Ontario College of Social Work and Social Service Workers or an equivalent college
Three (3) years’ experience providing clinical service
Valid class “G” driver’s license and own reliable vehicle
Clear drivers’ abstract
Clear vulnerable sector criminal record check
Preferred
Standard Cardiopulmonary Resuscitation (CPR), Automated External Defibrillator (AED), and First Aid Training
Applied Suicide Intervention Skills Training (ASIST)
Mental Health First Aid (MHFA)
Non‑Violent Crisis Intervention Training (CPI)
Experience with charting in Electronic Medical Records
Anishinaabemowin speaker
Knowledge
Knowledge of current mental health issues relevant to First Nations
Knowledge of protocols of First Nations on Manitoulin Island and surrounding areas
Knowledge of mental health services available within the Manitoulin Island catchment area
Knowledge of traditional healing practices of the Anishinabek
Skills
Skilled in a variety of mental health assessments, treatment planning and case management
Skilled in interpersonal relations
Strong time management skills
Excellent written and verbal communication skills
Demonstrated analytical and technical skills
Abilities
Solid ability to prioritize, plan, organize and schedule
Ability to work with minimal supervision
Ability to handle multiple tasks under pressure
Ability to work effectively as part of a team or as a leader when required to foster quality service to clients and to promote a good working environment
Ability to develop practices that ensure personal safety
