Working within an integrative primary care model, the successful candidate will contribute to an creative team of healthcare professionals committed to enhancing patient outcomes through evidence-informed practice and coordinated care delivery.

Provide the full scope of primary care services to patients of all ages.
Collaborate effectively within an interdisciplinary team workplace.

Qualifications:
Primary Care Nurse Practitioner Certificate (Extended Class)

Experience in a community-based primary care setting
Proficiency with Electronic Medical Record (EMR) systems
