Starting Salary Range: $69,000.00-$84,000.00

Position Overview

The Social Worker is a community based, cross-program role based out of the Health Centre. This position works
proactively and collaboratively across all health and wellness programs to support community members. The
primary responsibility of this role is to ensure that Mi’kmaw children are supported in ways that maintain their
cultural connections, identity, and community ties. The Social Worker role provides prevention-focused, early-
intervention, and wrap-around supports, helping community members navigate systems, access services, and
strengthen safety, stability, well-being and family connections. The role works closely with health staff and
external partners to reduce barriers, prevent crises, and support families in culturally grounded ways.

Key Responsibilities

- Provide Case Management through a strengths-based, trauma-informed approach. Complete intakes,

assessments, and collaborative support plans focused on prevention, wellness, and a continuum of care.

Provide short-term and ongoing case management support, including advocacy, referrals, and system

navigation.



Support clients experiencing complex needs, including mental health challenges, substance

use, housing instability, family conflict, or involvement with multiple service systems.

- Prevention & Family Support by proactively working with families to reduce risk of child welfare

involvement and strengthen protective factors. Support parents and caregivers with goal-setting,

life skills, problem-solving, and connection to culturally appropriate supports. Collaborate with

prevention, wellness, and community programs to deliver integrated supports.

- Collaboration & Coordination by working closely with Health Centre staff across programs to ensure

coordinated, wrap-around care. Liaise with external service providers, including community programs,

and provincial services. Participate in case conferences, care planning meetings, and multidisciplinary

teams as required.

- Cultural Safety & Advocate - Promote culturally grounded approaches that honour Mi’kmaq identity,

family systems, and community strengths. Advocate for clients within external systems to ensure

culturally safe, respectful, and equitable services. Support reconnection to culture, and community as

part of overall case planning.

- Documentation, Reporting & Accountability - Maintain accurate, timely, and confidential case notes and

documentation in accordance with policy and ethical standards. Contribute to program data collection,

reporting,



and evaluation.

Qualifications & Requirements

Education & Certification

- Bachelor’s or Master’s Degree in Social Work (BSW or MSW).

- Registered and in good standing with the Nova Scotia College of Social Workers (NSCSW).

- Mental Health First Aid training (can provide)

- Non-Violent Crisis Intervention training (can provide)

Experience
• 3-5 years of experience providing direct social work services, ideally in community-based, health, or

Indigenous settings.

- Experience working with families, children, youth, and adults with complex needs.

- Knowledge of child welfare, mental health, addictions, and social service systems (asset).

Skills & Attributes
• Strong relationship-building, communication, and advocacy skills.

- Ability to work independently while contributing effectively to a multidisciplinary team.

- Sound ethical judgement, professionalism, and respect for confidentiality.

- Strong organizational and documentation skills.

- Mi’kmaq language and/or lived experience in First Nations communities considered a strong asset.

Additional Requirements
• Valid driver’s license and access to reliable transportation.

- Explicit Criminal Record Check (Vulnerable Sector).

- Clear Child Abuse Registry Check.

- Valid CPR & First Aid (or willingness to obtain).
