Job Status: Full-Time
Reporting To: Prevention Programs Manger
Salary Range: $60,000 - $65,000

Position Overview

The Case Manager – Prevention & Post-Majority Support Services provides prevention-focused case
management to two groups: youth and young adults ages 19–25 transitioning from care, and families needing
added support to prevent formal child welfare involvement. The role offers early support, coordinated services,
and more intensive help when needed, connecting people to housing, education, wellness, cultural, and
community supports. Working closely with the Prevention Programs Manager and service partners, the Case
Manager helps build stability, strengthen family and community connections, self-determination, and reduce
the need for agency involvement in a way that reflects Mi’kmaq values.

Key Responsibilities Summary

- Provide prevention-focused case management

- Engage and support post-majority youth and young adults

- Deliver wrap-around, coordinated supports

- Develop & implement appropriate workshops, initiatives and events

- Strengthen connections to family, culture, language, and community

- Provide and manage financial and practical supports

- Ensure strong program accountability.

Qualifications





Education & Certification (any of the following):

- Post-secondary diploma or degree in Social Services, Human Services, Child and Youth Care, Community

Development, or a related field (or an equivalent combination of education and experience), or an
equivalent combination of relevant training and work experience

Experience

- Experience working with youth and young adults, including transition planning and case management;

experience supporting youth leaving care is an asset.
- Ability to work in a respectful, strengths-based, and culturally safe way, including supporting individuals

or families impacted by mental health, substance use, or trauma.
- Knowledge of (or willingness to learn) ISC Post-Majority Support Services requirements, and experience

working in Mi’kmaq or other First Nation communities or community-based health, wellness, or social
services settings.

Skills & Attributes

- Strong relationship-building and communication skills, with the ability to listen well, handle challenging

situations, and support and advocate for people in a respectful and confidential way.
- Solid case management and organizational skills, including intake, planning, documentation, and

managing multiple cases independently while working well as part of a multi-disciplinary team.
- Knowledge of mental health, addictions, and community supports, comfortable using basic computer

systems, and a commitment to learning, growth, and building a healthier community.

Other Requirements

- Valid driver’s license and reliable transportation
- Willing to complete a Criminal Record Check (with Vulnerable Sector Screening) and Child Abuse

Registry Check

- Willing to attend training and events outside of regular hours when needed
- Valid First aid & CPR (willingness to obtain)
