Registered Practical Nurse

Reports To

Manager, Clinical Services

Summary The Registered Practical Nurse (RPN) in a primary care setting plays a key role in delivering efficient, patient-centered care within a fast-paced clinical environment. Working closely with physicians and the interdisciplinary team, the RPN provides essential clinical support by preparing patients for assessments, assisting with procedures, and helping to manage patient flow throughout the day. Additional priority areas of the RPN role include preventive care and health promotion, administering immunizations across the lifespan, and supporting patient education to encourage healthy behaviors.

RPNs are also actively involved in chronic disease management, supporting patients with conditions such as diabetes, hypertension, and COPD. They assist with monitoring, follow-up care, and reinforcing care plans to help patients achieve better health outcomes. RPNs contribute significantly to timely access, continuity of care, and improved patient experience in primary care.

Work Location: in-person at either Rockwood or Erin clinics, with virtual opportunities

Requirements

· Proven ability to work collaboratively as part of a highly interdependent team, ideally involving health professionals.

· Strong interpersonal and organizational skills

· Robust nursing assessment skills for primary care setting

· Excellent performance of medications administration, including injections and preferred venipuncture skill

· Exceptional communication (written and oral),



decision making and problem-solving skills with the critical ability to multi-task, set priorities and meet deadlines

· Proficiency and experience working in a computerized environment.

· Sound judgment, tact and diplomacy with an emphasis on discretion and confidentiality

· Flexibility required to work in a rapidly changing environment

· Broad knowledge of community/primary health care and an appreciation of the requirements of rural health care provision

· Familiarity with East Wellington and the spectrum of health and social services available

· Class G Ontario driver’s license, access to a car, and the ability and willingness to drive between clinics and, otherwise, as is occasionally necessary

· In good standing with the college of nurses as a licensed registered practical nurse

· Proof of up-to-date immunization, CPR/AED Level-HCP, N95 mask fit test, and vulnerable sector check
