Job Description: Provides ongoing mental health and psychosocial client services. Services delivered include screening and assessing client needs and determining relevant treatment plans. Initiates or participates in community development initiatives, participating in PHC initiatives evaluation within site.

n

Promotes quality health care outcomes working within established standards of practice to ensure that clients receive a high standard of primary health care. This will be done in accordance with organization mission statement, policies and procedures.

- Experience working with clients and families coping with mental health disorders

- Bachelor degree in Social Work, Psychology or other health related discipline

- Ability to conduct a suicide risk assessment with other health care providers

- Knowledge and skill in mental health intervention methodologies.
