Job Description

- Providing single point of contact for clients discharging within the SHA.
n
- Providing single point of contact for clients transfers, admissions and discharges with SHA.
n
- Coordinating the utilization of the electronic assessment system RAI-MDS (hereafter referred to as MDS) as the Screening tool for Long Term Care eligibility and placement and alternate level of care services placement in the region.
n
- Maintaining the electronic bed utilization system for IRH and the SHA.
n
- Maintaining the electronic bed utilization system for the region.
n
- Providing an effective program and continuous quality improvement and management practice to ensure effective utilization of the system in consultation with Managers and Directors of Rural Health Services.
n
- Maintaining data integrity and data submission to the region as required.

Education

- Bachelor of Science in Nursing (BScN)
n

Licenses

- Basic Life Support Certificate (BLS)
n

Other Education and Training

- Nursing diploma is acceptable

- Bachelor of Psychiatric Nursing is acceptable

- Psychiatric Nursing diploma is acceptable
