Job Description
The Primary Health Care (PHC) Registered Nurse (RN) is an integral member of the primary health care team, providing and coordinating initial, continuing and comprehensive nursing services to patients in the PHC sites. The PHC RN promotes quality health care outcomes working within established standards for case management practice to ensure that patients/clients receive a high standard of primary health care. This will be done in accordance with Sunrise Health Region mission statement and Integrated Primary Health Services policies and procedures of the facility and department. The PHC RN initiates and/or participates in community development initiatives within the site. The PHC/RN reports to their respective Primary Health Care manager.n
Experience
Two (2) years advanced RN (NP) in applicable area
Education
Bachelor of Science in Nursing (BScN)
Other Education and Training
Basic Life Support Level C Certificate (BLS‑C)
Knowledge and Abilities
Ability to take initiative
Ability to communicate with clients from diverse cultural backgrounds
Demonstrates ability to accept and implement change
Knowledge in applicable health promotion and health prevention initiatives
Ability to work independently and as a member of a multi-disciplinary team
Demonstrated clinical competency
Demonstrated understanding of the prevention and management of chronic diseases
Proficient in case management skills and client follow-up

Registered/eligible with the College of Registered Nurses of Saskatchewan (CRNS)
Kamsack is located approximately 83 km northeast of Yorkton
Travel within the region and provincially
Nursing experience in Chronic Disease Management is desired.
