In this part time position, you will report to the Care Manager and be responsible for directing client care under the home care program. You will work closely with the interdisciplinary team to evaluate client needs and implement appropriate resources. Essential skills include wound care applications, palliative support, and case management in various community settings.
Key Responsibilities:
• Assess client needs for personalized care plans
• Coordinate interdisciplinary care and resources
• Facilitate client and family education on care processes




• Provide direct care, including wound management
• Handle on-call assignments as needed
Requirements:
• Accreditation from an approved nursing program
• Active or eligible registration with CRNA
• Current BCLS-HCP certification
• Completion of mandatory online courses for lapsed nurses
• Ability to perform physically demanding tasks
