We are recruiting for a SERVICE COORDINATOR for Here 24/7

- There are three opportunities available to fill existing vacancies:

- One regular full-time position (35 hours per week - Thursday, Friday and Saturday 6pm to 6am plus one additional shift per month 8:00pm to 6:00am on the second Sunday of the month) based at 80 Waterloo Ave. Guelph.

- One regular full-time position (35 hours per week - Thursday and Friday 7pm to 7am and Saturday 8pm to 8am with one additional shift per month 8pm to 6am on the second Wednesday of the month) based at 80 Waterloo Ave. Guelph.

- One contract full-time position until January 31, 2027 (35 hours per week - Tuesday to Saturday 5pm to 1am) based at 55 King St. W. Kitchener.

*These opportunities currently follow a hybrid work model. Shifts during core daytime hours (6am-12am) are typically worked on-site, while overnight and weekend shifts may be worked remotely. As service needs evolve, some weekend and statutory holiday coverage will transition to on-site work during daytime hours. The ability to work a hybrid schedule is determined by operational requirements, and the Agency reserves the right to modify work arrangements, including hybrid schedules.

Reporting to the Team Lead, Here 24/7, the Service Coordinator provides a range of services at the “front door” to the mental health and addictions system, including intake, referral, crisis response functions, and brief support to individuals awaiting ongoing services. Service Coordinators are part of a large, integrated team serving Waterloo and Wellington, while working within smaller, geographically based hubs to respond to and meet local service needs. Successful incumbents will need to go mobile as necessary based on the needs of the person and risk issues. The Service Coordinator also supports Ontario Health West’s Provincial Coordinated Access model by completing standardized screening and triage,



managing referrals, and matching individuals to the most appropriate mental health and addictions services across the system. This role works in close alignment with the 988 Suicide & Crisis Lifeline, supporting coordinated crisis pathways, warm transfers, and follow‑up where appropriate, as well as with community‑based crisis and stabilization services, including HART Hubs (Homelessness and Addiction Recovery Treatment Hubs).

Responsibilities include:

Crisis Intervention:

- Respond to individuals presenting in crisis, either in person or over the phone

- Provide mobile crisis response in the community as required

- Assess urgency, risk factors, strengths, resources and needed services

- Initiate referral to internal and external community agencies following stabilization of crisis

- Liaise with police, hospital staff, Family and Children's Services and other community supports to ensure safety and support of children/adolescents in crisis

- Provide short-term crisis support services to facilitate a transition to other supports

Referrals:

- Respond to referrals and assess support needs

- Connect with individuals referred for mental health & addiction services, assess for support needs including desired outcomes

- Facilitate links to community agencies and service

Wait List:

- Maintain regular contact / follow-up with individuals waiting for services

Service Information:

- Receive and act on inquiries about services

- Make referrals to other appropriate service providers as relevant

Qualifications:





- Degree/diploma in psychology, social work, or relevant health or social science field

- Three or more years of related experience

- Well-developed intake, referral, crisis assessment, intervention and brief support skills

- Ability to work well under pressure in a team environment and also independently

- Knowledge of a broad range of mental health, addiction and concurrent issues

- Knowledge of our system’s mental health and addiction supports and resources

- Valuable organizational, communication, interpersonal and problem-solving skills

- Ability to work collaboratively in a busy inter-disciplinary setting and with formal and informal community resources

- Well-developed knowledge base of the various service systems within the catchment area

- Demonstrated working knowledge of personal computers, database and word processing applications

- Demonstrated belief in the values and philosophy of CMHA WW

- Demonstrated sound clinical and practical judgement

- A valid driver’s license, vehicle and insurance, as travel is required

CLASSIFICATION AND RATE OF PAY: Level 5 (OPSEU) - $33.07 to $40.30 per hour
