Become a vital member of our Transitional Care team as a part time Registered Nurse. Provide compassionate, patient-focused care, assessing and implementing strategies to meet diverse needs in a supportive environment.
In this role, you'll work within a collaborative healthcare framework, focusing on enhancing the quality of life for patients in non-acute settings. You will uphold high standards of clinical practice while empowering individuals through integrated care strategies. Engage actively with patients and families to meet their holistic needs across all dimensions.
Key Responsibilities:
• Assess and implement patient-centered care plans
• Collaborate effectively with healthcare professionals




• Ensure the privacy and security of patient information
• Serve as a mentor to nursing peers and students
• Support ethical practices in nursing care
Requirements:
• Valid, unrestricted RN license from Ontario
• Current CPR certification required
• Nursing Diploma or BNSc preferred
• Commitment to patient-centric approaches
• Strong collaborative and communication skills
