Nurse Practitioner

Locations: Mental Health & Addictions Campus and Transition, Charlottetown

Mental Health & Addictions Acute and Transition, Prince County Hospital

The Nurse Practitioner manages a patient caseload as an integral member of a Health Care Team. The team is composed of a variety of health care providers and team composition may vary dependent on identified needs. The Nurse Practitioner works collaboratively with all health care providers. Direct client services provided by the Nurse Practitioner include client medical assessment, diagnosis and treatment of minor illness, health promotion and chronic disease prevention and management. This position is responsible for acute care and transitions sites allowing the NP to follow the patient care pathway until discharge. Works collaboratively with the Most Responsible Provider (MRP) in Acute Mental Health, In Patient Withdrawal Management and Child and Youth units. The Nurse Practitioner is the MRP for the Transition’s Addictions Unit. The Nurse Practitioner is the MRP for the Transition’s Addictions Unit. The Nurse Practitioner supports clients in Extended Care Housing, Brackley Stables and Small Options by providing regularly scheduled clinics.

Duties:

- Perform comprehensive, focused health assessments including a health history and physical examination using a holistic framework and multiple assessment techniques relevant to a client's stage of development, culture, health and health problems.
- Analyze client symptoms and assessment data by applying the principles of pathophysiology, pathogenesis, epidemiology, demographics and relevant research
- Discuss with and involve clients in decision making regarding their health, and in taking responsibility for their health
- Recognize and initiate effective interventions, consultations and referrals in emergent and unstable client situations
- Triage and direct clients and use a collaborative approach at all times
- Initiate referral of client to another service site when indicated by acuity or physician availability and stabilize clients until transfer as necessary
- Monitor, adjust, and document the outcomes of drug therapy, interventions and treatment plans: detect actual and potential adverse reactions,



and intervene appropriately;
- Monitor and document client's health status and outcomes.
- Provide direct guidance and counselling on health promotion and illness prevention strategies in accordance with client health status and needs
- Collaborate with clients, interdisciplinary teams, and community members in assessing health promotion, illness prevention needs and provide input as requested for health promotion and illness prevention programs use, strategies that address the broad range of health determinants in developing plans to promote health and prevent illness and injuries
- Duties may include some after-hours coverage on a rotational basis
- Provides coverage for the other divisional Nurse Practitioners when required

Minimum Qualifications:

- Graduation from an approved and recognized nurse practitioner program.
- Active or eligible for licensure as a Nurse Practitioner with the College of Registered Nurses and Midwives of Prince Edward Island.
- Maintain CPR certification
- Thorough knowledge and proficiency in medical and nursing physical assessments and treatment methods. Critical thinking proficiency to assess, analyze, diagnose, select treatment, evaluate, and refer patients with common acute or chronic health conditions, or for health promotion/illness prevention.
- Knowledge of practice setting programs and the community it serves, as well as current grasp of evidence­ based approaches and epidemiology issues pertinent to the catchment area and available resources.
- Previous Nurse Practitioner experience in a mental health setting preferred.
- Five or more years nursing experience in a combination of hospital and community settings.
- Previous experience in leadership roles considered an asset.
- Proficiency and experience working in a computerized environment.




- Skilled in independent performance of advanced elective procedures such as removal of lesions including nevi (moles), warts or toenails, suturing, suture removal, staple removal and wound assessment and care, wound debridement, ear syringing, examinations including Pap smears, bi-manual pelvic exams, breast exams, digital rectal exams, cytology collection, application of plaster back slabs, fitting of cervical diaphragms, initiation and discontinuation of peripheral intravenous infusions, initiation and discontinuation of urinary catheters, incision and drainage of external abscesses and boils, insertion, monitoring and replacement of vaginal estrogen ring and pessaries, ring blocks and venipuncture.
- Skilled in provision of autonomous coverage as initial point of entry for clients seeking outpatient care within the practice setting.
- Performs to maximum scope of practice; recognizes acuity/presentations beyond scope and utilizes cooperative approach when necessary.
- Demonstrated ability to work effectively in an interdisciplinary environment with excellent collaborative, team building and consultative skills.
- Excellent communication, facilitation and interpersonal skills in dealing with clients, families, community groups, professional colleagues, and referral partners.
- Proven verbal and written communication skills to ensure an effective and efficient working environment.
- Demonstrated ability in preparing and presenting to the community and practice setting staff information for the promotion of health and/or partnership.
- Strong analytical, problem-solving and decision-making abilities.

External candidates may be eligible to receive a $8,000 or $18,000 Nurse Practitioner Incentive, plus a $5,000 Rural or $8,000 Student Loan Incentive depending on location of employment. To be eligible for recruitment incentives candidates must hold a 1.0 FTE, this can be a full-time role or a combination of part-time positions. Relocation Assistance may be available to part or full-time candidates.

Salary Range: $57.94 - $65.59 per hour

Bi-Weekly Hours: 75 Hours Bi-Weekly / Days with Some After Hours as Required
