Coordinate HIV/HCV treatment access with compassionate case management skills in this part-time, 24-hour-per-week position at Hollyburn Family Services.The Link to Treatment Care Coordinator supports newly diagnosed individuals while coordinating care services through a harm reduction philosophy. This position develops care plans, supports mental health and addiction needs, and connects individuals with health, housing, income, and social services.
