Deliver community nursing care with ParaMed in Guelph, using patient assessments and care plans. Work full time, mornings, including every third weekend.

As a General Visiting Registered Nurse, you will visit patients of all ages in various settings. You will assess individualized needs, monitor care plans, and build therapeutic relationships. The position supports patients remaining comfortable at home, with scheduled visits and required after-hours on-call coverage.

Key Responsibilities:

- Assess individualized patient needs using appropriate clinical tools
- Execute and continuously monitor patient care plans
- Build rapport and trust through collaborative patient engagement
- Maintain and update qualified patient records
- Provide required after-hours on-call coverage for patient safety

Requirements:

- Current CNO registration as a Registered Nurse in good standing
- Valid driver's license and access to a vehicle
- Willingness to travel between patient residences
- Reliability and flexibility for scheduled community visits
- Community care experience is an asset
