NLX Health seeks a Health and Wellness Navigator who is a Registered Social Worker. The role supports wraparound care through holistic assessments, service coordination, wellness outreach, and clinical referrals. You will work with mental health clinicians, nurses, physicians, and wellness programs while reporting to the Psychiatric Nurse Manager.

Key Responsibilities:

- Serve as the primary outreach and navigation contact
- Conduct holistic assessments and guide individuals through supports
- Apply culturally safe, trauma-informed approaches
- Coordinate FNHA-funded services, treatment,



and harm reduction supports
- Maintain documentation and support reporting requirements

Requirements:

- Registered Social Worker in positive standing with BCCSW
- Clinical mental health or substance use experience
- Understanding of Indigenous health and wellness frameworks
- Excellent communication, organizational, and interpersonal skills
- Valid Class 5 license and reliable vehicle

Use social work, outreach, and care coordination skills to connect community members with culturally safe wellness services.
