This full-time visiting nursing role serves patients with differing complexity of palliative care needs. You will assess individualized needs, execute care plans, and monitor changes. Building rapport supports therapeutic relationships and helps patients remain comfortable at home.
Key Responsibilities:
- Assess individualized patient needs using appropriate techniques
- Execute and continuously monitor patient care plans
- Build rapport and trust through cooperative engagement
- Maintain and update professional patient records
- Provide scheduled visits in various patient settings
Requirements:
- Experience in Pediatrics and Palliative care
- Registration with CNO as a Registered Nurse in good standing
- Valid driver's license and access to a vehicle
- Willingness to travel within a designated area
- Required after-hours on-call coverage for patient safety
