Bridges Community Health Centre (CHC) is a non-profit, community-governed organization funded by the Province of Ontario. Our holistic, interdisciplinary model of health and wellbeing focuses on primary health care, health promotion, illness prevention, and community capacity building, while considering underlying social determinants of health and barriers to accessing care. Our greatest strength is our team, who is committed to the work they do, serving and strengthening the communities of Fort Erie, Port Colborne and Wainfleet. We are currently recruiting a full time Health Promoter to join our team.

JOB SUMMARY:

The Health Promoter addresses community health needs by focusing on creating, implementing, and evaluating health promotion, community, and advocacy initiatives in Fort Erie. In addition, the Health Promoter implements Bridges CHC’s communications activities. This position will generally be based out of the Fort Erie site.

QUALIFICATIONS REQUIRED:

- A master’s degree in public health, health promotion/education or a combination of three years of related work experience and relevant undergraduate education.
- Ability to work effectively with diverse populations, independently and as part of an interdisciplinary team.
- Proven skills in group facilitation, needs assessment, program planning, program co-design, implementation, and evaluation.
- Demonstrated ability to work effectively and collaborate with community members and partner organizations to address issues affecting their health.
- Excellent verbal and written communication and presentation skills.
- Excellent, group facilitation, interpersonal, conflict resolution, and relationship-building skills.
- Experience working in various social media platforms for health promotion activities.
- Experience in creating communication materials using Canva, Microsoft Office, Squarespace, and WordPress.
- Commitment to continuous learning, accuracy, and professional development.
- Thorough knowledge and understanding of community services and resources in Fort Erie, Port Colborne, and Wainfleet and broader Niagara region.




- Knowledge of Bridges Community Health Centre, the communities of Fort Erie, Port Colborne, and Wainfleet, and/or a second language that is reflective of the communities being served are all preferable assets.

ADDITIONAL REQUIREMENTS: · Ability to work autonomously in a fast-paced environment. Ability to work flexible hours, including occasional evenings and weekends.

· Valid Driver’s License and access to own vehicle required.

KEY RESPONSIBILITIES:

Community Development and Health Planning, Implementation and Evaluation:

· Consult with community partners, and community members to identify community health needs through needs assessments, environmental scans, focus groups, surveys.

· Identify potential community partners to develop a team-oriented response to the health needs of the community.

· Co-design and implement a service plan with community partners, clients, and community members to meet identified community needs.

· Engage community partners, members, and clients in the evaluation of community initiatives and health promotion activities and share feedback with all involved.

· Collaborate with the Bridges CHC interdisciplinary team to plan, deliver, and evaluate integrated health promotion initiatives which align with and support Brides CHC’s Strategic Plan.

. Identifying, developing, and facilitating in-person and virtual programs needed to support the health and well-being of Bridges Community Health Centre (CHC) clients and community residents in Fort Erie, Port Colborne, and Wainfleet.

· Educate the community about the Community Health Centre model of care and promote programs and services.

· Organize and promote health promotion events.





· Under the direction of the Executive Director implement advocacy initiatives.

· Maintaining up-to-date, accurate and appropriate documentation related to encounters with clients and group programs (as required by our funders) in Bridges CHC’s electronic medical record system.

Communications:

· Collaborate with the leadership team in organizing, creating, and publishing content on social media platforms to promote Bridges CHC’s programs, services, and initiatives; ensure social media channels are up to date; and post content regularly.

· Conceptualize and design visually engaging communication materials such as newsletters, program posters, presentations to community partners, reports etc.

· Build and maintain a communication network with partners, distribute communication materials, and maintain the distribution list.
