Assess patients receiving home peritoneal dialysis Educate patients and families about home dialysis Coordinate appointments and community home visits Prepare circuits and program dialysis machines Monitor treatments and provide interventions with RN collaboration Current College of Nurses of Ontario registration in valuable standing Experience processing physician orders Access to a vehicle for community transportation Bring nephrology nursing practice, patient education, and therapeutic support to Halton Healthcare's community home dialysis program.
