ParaMed community health nursing offers scheduled visits, varied patient needs, and professional care documentation for a part time Registered Nurse. Location: County Area.

This position serves patients of all ages across the Erie St. Clair District and supports comfort at home. The Registered Nurse assesses individualized needs, executes and monitors care plans, builds therapeutic trust, and maintains records. Community care experience is an asset.

Key Responsibilities:

- Assess patients using suitable assessment tools and techniques
- Continuously monitor individualized plans of care
- Build collaborative relationships grounded in rapport and trust
- Update records after patient encounters and plans
- Cover required on-call periods, including after hours

Requirements:

- CNO Registered Nurse registration maintained in valuable standing
- Access to a vehicle and valid driver’s license
- Willingness to travel to patient residences
- Reliability and flexibility in community care scheduling
- Passion for making a difference in your community
