Deliver Indigenous health counselling and care coordination while connecting clients with Fraser Health and community services through collaborative practice.

This clinical Social Worker participates in a multidisciplinary Indigenous Health Liaison Team across hospital and community settings. You will plan, organize, and evaluate client needs and services, drawing on clinical counselling, social work, and Indigenous health knowledge. The position collaborates with First Nations Health Authority, Metis Nation BC, Fraser Health programs, and community partners to support referrals, education, and continuity of care.

Key Responsibilities:

- Plan, organize, and evaluate clinical and psychosocial services
- Provide health education supporting whole-body health and self-care




- Facilitate connections between clients and service providers
- Represent Indigenous Health with staff and community agencies
- Prepare reports according to established standards and procedures

Requirements:

- Knowledge of Social Work and Clinical Counselling principles
- Ability to assess, plan, implement, and evaluate
- Robust referral and community collaboration skills
- Demonstrated teaching skills and accurate documentation
- Physical ability to perform position duties
