Support medically complex clients as a registered Medical Social Worker coordinating care, counselling, and system navigation. Work rotating days and evenings in Dawson Creek.

This position provides social work services that maintain or improve client health, social well-being, and functioning. You will coordinate treatment, collaborate across disciplines, and connect clients and families with relevant specialized resources. The role also includes education, advocacy, group work, and facilitating self-management.

Key Responsibilities:

- Assess designated clients and develop responsive care plans
- Coordinate treatment across multidisciplinary health care teams
- Connect clients with specialized complex care resources
- Teach and coach individuals and groups




- Organize services supporting client-focused, person-centred care

Requirements:

- Bachelor’s Degree in Social Work from an accredited university
- Two years recent healthcare experience with concurrent disorders
- Registration with the British Columbia College of Social Workers
- Valid BC driver’s license
- Physical ability and demonstrated Windows computer skills
