As a Nurse Case Manager, you will coordinate services for pharmaceutical patient support programs and manage assigned cases. Responsibilities include clinical monitoring, data collection, follow-up, compliance tracking, and adverse event reporting. You will also work with healthcare providers, pharmacies, infusion services, and reimbursement specialists to support coordinated patient care.

Key Responsibilities:

- Document and report adverse events under pharmacovigilance requirements
- Collect data and maintain accurate case records
- Monitor compliance, discontinuation, and therapy disruption




- Coordinate patient services with pharmacies and infusion providers
- Develop training materials for new program team members

Requirements:

- Registered Nurse in good standing with provincial regulator
- Two to four years of clinical experience
- Patient Support Program experience is a solid asset
- Immunology experience is an asset
- French and English bilingual communication skills are preferred
