As Clinical Nurse Lead, you will work independently without direct reports while serving as a clinical resource for nursing colleagues. Your work includes complex patient assessment, case-management coverage, onboarding, service coordination, documentation review, and operational improvement. Occasional travel includes conferences and client meetings in Burlington, Ontario.

Key Responsibilities:

- Assess complex situations and guide appropriate clinical escalations
- Coordinate services with patients, physicians, pharmacies, and internal partners
- Provide versatile case-management coverage during operational needs
- Review cases for completeness, consistency,



and program alignment
- Support quality reviews, projects, technology implementation, and training

Requirements:

- Active registration with the applicable Ontario nursing regulatory college
- Registered Nurse or Registered Practical Nurse designation
- Four or more years of relevant nursing experience
- Independent work style with sound clinical judgment and prioritization
- Ability to perform extended computer-based work and virtual meetings
