This supervisor supports clients within the community and helps ensure ethical, quality care across home health services. You will evaluate clinical interventions, coach staff, address concerns, and collaborate with nursing colleagues. Community nursing knowledge, documentation standards, and strong problem-solving skills are central to the work.

Key Responsibilities:

- Provide staff education, coaching, mentoring, and problem-solving support
- Engage patients in collaborative care planning and health teaching
- Build rapport while identifying patient needs, strengths, and goals




- Maintain practice knowledge affecting clinical scope
- Coordinate incident investigations, documentation, and required timelines

Requirements:

- RN or RPN registration with the College of Nurses of Ontario
- At least five years practicing as an RN or RPN
- Understanding of community nursing practice principles
- Excellent organizational, interpersonal, and teaching skills
- Current clear Vulnerable Sector Screening
