The physician will provide comprehensive family medicine services, including assessment and management of acute and chronic medical conditions, preventive care, routine physical examinations, health promotion, medication management, referrals and coordination of care. Duties will include maintaining appropriate clinical documentation, reviewing investigations and results, responding to patient communications, and providing longitudinal follow-up to an attached patient panel. Physicians may also provide same-day/urgent assessments as appropriate to the practice model and participate in cooperative care with other healthcare professionals.
