Vancouver Coastal Health (VCH) in Vancouver is seeking a Social Worker to support Public Health teams in Vancouver and Richmond. You will work within a client- and population-centered care model as part of an interdisciplinary team, providing consultative social work services to a designated population group.

You will conduct comprehensive psychosocial assessments, prepare case histories, deliver psycho-education and interventions, and coordinate individualized treatment and discharge planning,
