Barriere Health Centre seeks a Community Integration Care Coordinator supporting day-to-day operations and quality client care. The role coordinates client movement through healthcare systems, accesses resources, and supports client and family centered care. It also provides clinical guidance, counselling, education, and interdisciplinary coordination.

Key Responsibilities:

- Coordinate daily operations within designated clinical areas
- Support seamless client movement through the healthcare system

Requirements:

- Graduate from an approved School of Nursing
- Hold current BCCNM practicing registration
