Apply psychosocial assessment and crisis intervention skills as a Social Worker II supporting Covenant Health’s Emergency Department Clinical Triage Team. This temporary part-time role serves patients and families.

Covenant Health is recruiting a Social Worker II for a one-year pilot improving timely access, patient flow, and connections to services. You will collaborate with physicians, nurse practitioners, registered nurses, and the broader Emergency Department team. The work includes assessment, brief intervention, crisis support, resource navigation, advocacy, and discharge planning.

Key Responsibilities:

- Identify psychosocial needs and barriers to care
- Provide psychosocial assessments and brief interventions
- Support patients and families during crises




- Connect clients with healthcare and community services
- Facilitate safe, timely transitions and discharge planning

Requirements:

- Bachelor Degree in Social Work required
- Active Alberta College of Social Workers registration required
- Demonstrated comprehensive assessment abilities required
- Robust communication and conflict resolution skills required
- Minimum two years’ case management experience preferred
