Support respiratory rehabilitation patients as a part-time Certified Care Attendant at UHN’s West Park Healthcare Centre in Toronto. Provide fundamental care under Registered Nurse supervision.

This permanent part-time position serves Respiratory Rehab and Respiratory Complex Continuing Care patients. The Certified Care Attendant follows individualized care plans, supports daily routines, and promotes patient comfort. Duties include transfers, ambulation, hygiene, documentation, and reporting changes in condition.

Key Responsibilities:

- Follow KARDEX instructions for daily patient care needs
- Assist with meals, transfers, ambulation, toileting, hygiene, and grooming
- Report patient condition changes promptly to Registered Nurses




- Document care using Core Care Summary and Progress Notes
- Operate lifts and wheelchairs and report equipment concerns

Requirements:

- Grade 12 diploma or equivalent education
- Current HSCPOA registration
- Personal Support Worker certification from an approved program
- Preferably one year equivalent experience within three years
- Excellent verbal and written communication skills
