Programme Coordination Coordinate administrative processes that enable the care coordination model to function effectively. Resident Identification Support the collation of eligible resident cohorts from the cohort patient lists. Validate and maintain lists and programme registers. MDT Administration and Facilitation Ensure relevant stakeholders receive information in advance of meetings. Record decisions, recommendations, actions and agreed outcomes. Monitor completion of agreed actions and escalate delays where appropriate. Information Management Maintain accurate programme records and tracking systems. Ensure documentation is complete, accessible and shared appropriately. Support data collection for programme monitoring and evaluation. Stakeholder Liaison Act as a key point of contact for operational enquiries. Ensure information flows effectively across the MDT. Support service users and professionals in navigating programme processes
