LOFT Community Services seeks a Psychogeriatric Case Manager for its 24/7 Assisted Living-Dunn program. Provide direct support to seniors with responsible mental illness, addictions, cognitive impairment, and complex physical health challenges. Build community linkages supporting independence, dignity, and self-respect.

Key Responsibilities:

- Develop individualized care plans and connect clients with appropriate resources
- Conduct RAI and OCAN assessments with accurate documentation
- Provide counselling, crisis intervention, and service navigation
- Coordinate care with families, caregivers, teams, and providers




- Advocate for clients while maintaining confidentiality and professional standards

Requirements:

- Post-secondary education in Social Services, Human Services, or equivalent
- At least three years supporting vulnerable seniors and older adults
- Knowledge of community resources, counselling, assessment, and engagement
- Understanding of harm reduction and psychosocial rehabilitation principles
- Vulnerable sector check, vehicle access, and occasional flexible hours
