This Clinical UCP Supervisor position leads PSWs and HSWs caring for clients in the community. The supervisor assesses patient needs, evaluates interventions, and helps teams meet legislated standards, company policies, and clinical documentation requirements. Strong leadership, teaching, and problem-solving skills are central to the work.

Key Responsibilities:

- Ensure community practices comply with standards and policies
- Respond to concerns from staff, patients, families, and colleagues
- Engage patients in collaborative problem solving and care planning
- Build rapport while identifying patient goals and capacities




- Maintain current practice knowledge affecting scope of practice

Requirements:

- RN or RPN registration with unrestricted Ontario practice entitlement
- Five years of practicing Registered Nurse or Registered Practical Nurse experience
- Proven team leadership and management experience
- Excellent organizational, interpersonal, and teaching skills
- Clear current Vulnerable Sector Screening
