As a full time Home Support Supervisor, you will support Spectrum’s Integrated Care Program as an RPN. The position includes patient assessment, care planning, outcome monitoring, staff management, and multidisciplinary collaboration. You will help address patient concerns while tracking service and performance indicators.

Key Responsibilities:

- Travel to patients’ homes throughout the community
- Assess patient needs and formulate individualized care plans
- Evaluate progress toward established patient outcomes
- Manage staff, appraise performance, and implement progressive discipline




- Document concerns, feedback, complaints, and key indicators

Requirements:

- Current College of Nurses of Ontario registration in valuable standing
- At least three years of progressive leadership experience
- Direct supervision experience, preferably in community settings
- Valid driver’s license and reliable vehicle access
- Excellent English communication and report-writing skills
