Lead client support at I Have a Chance Support Services Ltd. with care planning, crisis intervention, and leadership responsibilities. This full time schedule runs Monday through Friday with additional hours possible.

The Client Care Manager coordinates personalized support for individuals with disabilities and complex needs. You will monitor service quality, guide staff, and work with families, healthcare professionals, and stakeholders. The position combines advocacy, direct support, reporting, and compliance oversight.

Key Responsibilities:

- Coordinate individualized care plans and wellness supports
- Provide crisis intervention and post-incident debriefing
- Coach employees and support required training




- Review daily reports and maintain accurate documentation
- Participate in on-call duties and residence spot checks

Requirements:

- Degree or diploma in a relevant human services field
- Four years’ related experience with the population served
- Valid driver’s license, reliable vehicle, and clean abstract
- Vulnerable sector and child intervention record checks
- Understanding of harm reduction and team-oriented teamwork
